Recovery Case Management provides mental health and substance use recovery services to individuals residing in RISE housing programs services in need of additional support. We also provide these services to individuals living in the community receiving RISE Care Management services.
Recovery Case Management recovery coaches works closely with each individual to support their recovery goals.
Recovery coaches meet with service recipients regularly. We coordinates with residential program staff or care managers to provide the best possible care.

Eligibility
- RISE Recovery Services are available to individuals who currently reside in a RISE Residential Program or receive RISE Care Management Services.
- Participation in Recovery Case Management is voluntary and can be discontinued at any time. Services are discontinued either by choice or when the individual is no longer in need of the additional support to maintain their recovery.
Referrals
Care Managers and other RISE Programs Staff may refer individuals currently participating in other RISE Programs for Recovery Case Management by filling out the RISE Support Services Referral Form. Once a referral is received, an assessment is done. This determines the areas of support needed for individuals to successfully maintain their recovery.
