Care Management
In 2023 RISE Care Management programs provided services to 420 individuals – ensuring that their medical, behavioral health, and social services needs were met through care management services.
The Care Management Team helped many individuals connect with community services to help them received the services they need for their physical and mental health needs. Child and Youth Care Management also helped families with children with paying bills and getting the children presents at Christmas time.

Residential Housing Programs
In 2023 our Residential Housing Programs served a total of 269 adults experiencing mental health, substance use, or other life challenges.

Adelphi Street Shelter
Since opening in June 2023 through December 2023, the Adelphi Street Shelter provided shelter to 106 individuals. The Shelter Advocates have assisted the individuals to engage in community services, such as advocacy and case management, housing referrals, daily living skills, managing medical and mental health appointments, benefits assistance, and obtaining legal documents.

RISE Community Outreach Court Team
Since its inception in October 2020, 107 individuals have participated in the The Saratoga Springs Community Outreach Court Program. The services that the RISE Outreach Court Coordinators provide assists individuals to be compliant with their Court Orders. As a result, there have been demonstrable impacts on both participant wellbeing and public safety.
