Care Management
In 2025 RISE Care Management programs provided services to 322 individuals – ensuring that their medical, behavioral health, and social services needs were met through care management services.

The Care Management Team helped many individuals connect with community services to help them received the services they need for their physical and mental health needs. Child and Youth Care Management also helped families with children with paying bills and getting the children presents at Christmas time.

Residential Housing Programs
In 2025 our Residential Housing Programs served a total of 247 adults experiencing mental health, substance use, or other life challenges.

Recovery Programs
In 2025 our Residential Housing Programs supported 123 adults with their recovery journeys.

At Hedgerow House, Hedgerow House Apartment Program and RISE Above Residential Rehabilitation Program, Staff assisted these individuals in obtaining and maintaining their sobriety as well as helping them develop plans to reintegrate back into the community.
The OASAS Outreach and Engagement Services (OES) was a pilot program under the Office of Addiction Services and Supports (OASAS) that focused on providing street-level outreach and engagement services to populations who typically have difficulty accessing substance use disorder treatment.

Adelphi Street Shelter
Since it’s opening in June 2023, the Adelphi Street Shelter provided shelter to 248 individuals experiencing homelessness. The Shelter Advocates assisted the individuals to engage in community services, such as advocacy and case management, housing referrals, daily living skills, managing medical and mental health appointments, benefits assistance, and obtaining legal documents.

RISE Community Outreach Court Team
During 2025, 30 individuals participated in the The Saratoga Springs Community Outreach Court Program. The services that the RISE Outreach Court Coordinators provide assists individuals to be compliant with their Court Orders. As a result, there have been demonstrable impacts on both participant wellbeing and public safety.
Since its inception in October 2020, the Community Outreach Court has provided services to 256 individuals experiencing homelessness.
