RISE Recovery Services are offered through several unique and specialized programs.
We believe each individual is unique and responsible for their own recovery.
We believe people are entitled to trauma-informed, person centered care in a safe and supportive environment.
Our recovery coaches work closely with each resident to facilitate a successful, substance-free transition back into the community.
Outreach and Engagement Services
Outreach and Engagement Services is a pilot program under the Office of Addiction Services and Supports (OASAS) that focuses on providing street-level outreach and engagement services in the community to populations who typically have difficulty accessing treatment.
Office of Addiction Services and Supports (OASAS) Licensed Programs
Hedgerow House is a substance use treatment community residence program providing services to individuals in recovery from substance use.
Hedgerow Apartment Program (HAP) is a supportive transitional apartment program providing services to individuals in recovery from substance use.
Referrals for adults 18 years or older are made to Hedgerow House and HAP by calling Hedgerow House to schedule an intake appointment at 518-885-3872.
Office of Mental Health (OMH) Licensed Programs
PSAP Recovery Apartments provide residential support services in transitional apartments to adults with co-occurring mental health and substance use diagnoses who need support to live independently while maintain their recovery.
Mental health professionals may refer adults 18 years old and older to PSAP Recovery Apartments by completing the Saratoga County Adult SPOA Referral Form or call 518-584-1205 for more information.
Recovery Case Management
Recovery Case Management provides mental health and substance use recovery services to individuals residing in RISE housing programs who are in need of additional support. We also provide these services to RISE Care Management recipients.
RISE Care Managers and other RISE programs staff may refer individuals for Recovery Case Management by filling out the RISE Support Services Referral Form.

RISE Recovery Services Programs provide services in Saratoga County, but we accept out of county referrals for most of our Recovery Services Programs.
