RISE Housing and Support Services has partnered with Adirondack Health Institute (AHI) to provide Health Home Care Management services in Saratoga, Washington, and Warren Counties. The Health Home provides oversight and care coordination to individuals and families on Medicaid.
Care Managers assist with referrals for medical care, dental care, social services, mental health, substance use treatment, financial benefits, and housing.
We work with individuals to clearly communicate their needs to service providers to ensure that medical, behavioral health, or social services are obtained.
We assist individuals and family members to develop personalized Care Plans specifically tailored to their needs. Care Plans are developed with an emphasis on preventative, primary medical care to keep people healthy.
We seek to strength each individual’s ability to navigate the maze of systems. Doing so assists them to access services, both now and in the future.
Services Provided

- 24-hour emergency on-call
- Trauma-informed, person centered care
- Monthly meetings with Child and Youth Care Managers
- Service coordination with community providers and natural supports
Eligibility
Reside in an eligible county: Saratoga, Warren, and Washington.
Initial Eligibility Criteria must include two chronic conditions:

- Mental Health Condition
- Substance Use Disorder
- Asthma
- Diabetes
- Heart Disease
- BMI over 25
OR HIV/AIDS
OR Serious Mental Health Diagnoses
Referrals
Adult Care Management Services are provided in the following counties: Saratoga, Warren, and Washington. Mental health professionals may refer adults to Adult Care Management, by submitting a AHI Health Home Community Referral Form or call for more information.
