As a proud member of the Saratoga North-Country Continuum of Care (SNC CoC), RISE works in partnership with our community partners to create lasting solutions to end homelessness in our communities.
The SNC CoC is a U.S. Department of Housing and Urban Development (HUD) designated collaborative planning body serving Saratoga, Warren, Washington, and Hamilton Counties, where member agencies work in close concert to keep the needs of individuals and families experiencing housing insecurity at the heart of every decision.

Together, the CoC sets local priorities for HUD funding, coordinates housing and supportive services, and guides a strategic, data-informed response to homelessness through its Planning Body and community partnerships. Committed to preventing and ending homelessness, the SNC CoC actively engages government partners, nonprofit organizations, housing developers, service providers, and individuals with lived experience to shape a responsive and inclusive system of care.
This work is supported by a full continuum of services, from prevention and outreach to emergency shelter, transitional and permanent housing, and the supportive services necessary to help individuals and families achieve lasting stability.
Not All Homeless is the Same
Not all homelessness looks the same, and the federal funding our Continuum of Care manages must follow specific U.S. Department of Housing and Urban Development (HUD) definitions in order to determine eligibility.

Our Continuum of Care (CoC) programs primarily serve individuals who are literally homeless or fleeing domestic violence. Those who do not meet HUD’s definition may still receive support through other funding sources and community programs.
HUD Funded Housing Programs
The HUD-funded housing programs our community partners provide strengthen our entire community. They stabilize our most vulnerable neighbors, reduce strain on emergency systems, bring federal dollars into our local economy, and support coordinated, data-driven solutions to homelessness.
The Types of HUD-Funding Housing Program CoC Partner Agencies Provide include:
- Emergency Shelter: Immediate, short-term housing that provides a safe place to stay for people experiencing homelessness, often including basic needs like meals and support.
- Rapid Re-housing: A short-term program that quickly helps people move out of homelessness into their own housing, with temporary financial help and support services.
- Transitional Housing: Temporary housing that provides a safe place to stay while people work toward stable, permanent housing.
- Permanent Supportive Housing: Long-term housing paired with ongoing support services for people who need extra help to live independently and stay housed.

Key Roles of the CoC:
The Saratoga North-Country Continuum of Care is led by a Board of Directors and made up of local organizations and community members working together to address homelessness in Saratoga, Warren, Washington, and Hamilton Counties. It provides a space for partners to share ideas, identify needs, and find solutions to better support people facing housing challenges. Each year, the CoC also applies for federal funding through HUD to help bring resources and services to the region.

- To respond to and end homelessness – The CoC works to prevent and end homelessness through a coordinated, community-wide approach that prioritizes rapid rehousing, supportive services, and data-driven decision-making to ensure individuals and families can quickly access stable, permanent housing.
- Oversee Coordinated Entry (CE) – CE system provides a single, streamlined process for individuals and families experiencing homelessness to access housing and supportive services. Through standardized assessments, CE prioritizes those with the greatest needs, ensuring that limited resources are allocated fairly, efficiently, and with a focus on long-term stability.
- Oversight of the Homeless Management Information System (HMIS) – HMIS is a secure, centralized database used by service providers to collect and share client information, helping coordinate care, track outcomes, and improve services for individuals experiencing homelessness.

Get Involved
The Saratoga North Country COC is accepting new member agencies. Members meet two times per year, in May and October. All meetings are virtual via Zoom at this time. Membership includes the right to vote on matters impacting the CoC, though non-members are both welcome and encouraged to participate in discussions.
